“Come in “said Dr.Martin, newly qualified heart surgeon, in response to the soft knocking on his surgery door. The door opened slowly as the doctor got up from his chair and walked towards the door to greet his new patient, Mr. David Waters, here today to discuss his forthcoming heart transplant operation, tentatively scheduled for next week at this very hospital.

As the doctor approached, the vapid smell of stale cigar smoke quickly reached his nose, stopping him in his tracks. “Oh my god”, the doctor muttered to himself in disbelief. The smell was so gross and disgusting he felt momentarily nauseous as the smell quickly infiltrated the clean sterile air of his surgery. There really was nothing worse than a patient smoking right before an appointment, he thought to himself, could anyone believe this idiot was actually scheduled for heart surgery next week?
