First I have a brilliant idea. I search frantically for a notebook to write down the idea before I forget. Most of the time I am not anywhere near my computer, so I often forget. Secondly, I focus on what I was doing when the idea hit and try to recall the idea. Most of the time this works. Then I jot down the idea, develop the characters, and make an outline. I begin writing, following my basic outline. Sometimes I have a 30 point outline, others have just ten points. Each of my points is a scene or important plot point. Basically, I play connect the dots.
About half of the time, I end up tossing my original outline as the story develops and go in a new direction.

I have a basic idea of what I want. Very basic. I contact a cover artist and tell them my idea, give them my synopsis, and wait to see what they come up with. I am very pleased with the designs my cover artist has done!

My kindle fire tablet. I have a different account on my phone, so sometimes I use my phone because I have different books on there.

There is no rhyme or reason for what catches my eye. IT could be from a FAcebook ad, it could be advertised as free, it could be something I happened across like those if you like this book, you might like. . . I read several different genres so I get all sorts of suggestions. To be honest though, most of the books that catch my eye are not the suggested ones.
